Open Question: emancipation!! i need help dealing with all of it!?

ok i have a friends mom who wants to take me in, due to the mental health issue my parents give me. I have a social worker but she says she can not help me at this moment. I live in Kansas and would like to know if i need to fill out an actual document, or if i just need to type of the reasons of why i need to be emancipated and show i have a job and also show that my parents are mentally abusing me, and i even have therapists that agree with me.
